  Complete and Submit the order form below. Championship hoodie orders will only be accepted from the team manager listed on the schedule unless prior written permission by the manager is submitted to the league office  
     
  Championship hoodies can only be ordered for players that have won Spring, Summer, and Fall titles in the same league during the same year and with the same team sponsor. Players that were only part of 1 or 2 of the 3 titles can only receive t-shirts located here.  
     
  Championship hoodies are produced by Curtis Hensman at Cutting-Edge Sports Apparel. If you have any questions or concerns, contact Curtis directly.
