Championship Team Shirts

Teams that win championships in any division are entitled to custom championship shirts and a championship plaque. Championship team shirts are custom ordered on a per team/per championship basis. The team has the choice of not only which color shirts they want, but the color of the print as well. As shown below, the red, white, and blue Softball America logo will be placed on the top line across the chest. The 2nd line will show the league, season, and year of your championship. Line 3 will show the team or sponsor name in block letters.

Championship Team Shirt Order Form

Championship shirt orders will only be accepted from the team manager listed on the schedule unless prior written permission by the manager is submitted to the league office. The deadline for customizing your shirt order is the end of the current calendar year. Championship shirt will not be ordered in the event the team manager fails to customize in a timely manner.
